Here is the Agreement as discussed with your counsel. It is fine to sign as Jane Doe. However, it should be witnessed by someone such as a counsellor. I need a copy with original signatures for my files. Please make arrangements to have it delivered to the above-noted address to my attention marked "confidential". I have made changes suggested by your counsel. I have also added a new line after item 4 on page 2 which should not cause difficulty. "Receipts need be provided as requested by police".
